What is Tagrisso? What is Osimertinib 80mg?

The drug Tagrisso (osimertinib) is a cancer drug that interferes with the growth and spread of cancer cells in the body.

It is used to treat a type of non-small cell lung cancer . This medication is for use only if your tumor has a specific genetic mark, which your doctor will examine.

Tagrisso is approved by the US Food and Drug Administration (FDA) on an "accelerated" basis. In clinical studies, tumors responded to this drug. However, more research is needed to determine if this drug may prolong survival.

Indications for drug treatment

The drug is a single therapy indicated for:

  • First-line treatment for adult patients with metastatic non-small cell lung cancer (NSCLC) locally by triggering mutations of epidermal growth factor (EGFR).
  • treatment of adult patients with mutagenic positive NSCLC or EGFR T790M metastasis.

How  Osimertinib works

  • Tagrisso works by blocking EGFR and can help slow or stop your lung cancer from growing. It may also help reduce the size of the tumor.
  • If this is the first protein kinase inhibitor you are getting, it means your cancer has an EGFR gene defect such as 'exon 19 removed' or 'exon 21 substitution mutation'.
  • If your cancer has progressed while you are being treated with other protein kinase inhibitors, it means your cancer has a genetic defect called 'T790M'. Due to this defect, other protein kinase drugs may no longer work.

Drug dosage Tagrisso

The recommended dose is 80 mg osimertinib once a day until disease progression or intoxication is unacceptable.

If a dose is missed, the dose should be taken up unless the next dose is due within 12 hours. Tagrisso can be taken with or without food at the same time every day.

Adjust the dose

Dose interruption and / or dose reduction may be required based on safety and individual tolerability. If dose reduction is necessary, then the dose should be reduced to 40 mg orally once daily.

Note: The intensity of clinical adverse events was classified according to the Common Language for National Cancer Institute (NCI) criteria for Adverse Events (CTCAE) version 4.0.

Special groups

No dose adjustment is required due to patient age, body weight, sex, ethnicity and smoking status.

Liver failure

  • No dose adjustment is recommended in patients with mild hepatic impairment (total norm of normal bilirubin ≤upper (ULN) and aspartate aminotransferase (AST)> ULN or total bilirubin> 1.0 to 1.5x ULN and any AST) or impaired Moderate liver (total bilirubin 1.5 to 3 times ULN and any AST) but caution should be exercised when administering TAGRISSO to these patients.
  • The safety and efficacy of this medicinal product have not been established in patients with severe hepatic impairment. Until additional data become available, use in patients with severe hepatic impairment is not recommended

CKD

  • No dose adjustment is recommended in patients with mild, moderate or severe renal impairment. Limited data are available in patients with severe renal impairment. 
  • The safety and efficacy of this medicinal product have not been established in patients with end-stage renal disease [creatinine clearance (CLcr) <15 ml / min, calculated using the Cockcroft and Gault formula], or hemodialysis. Caution should be exercised when treating patients with severe and end-stage renal failure

For children

The safety and efficacy of the drug Tagrisso in children or adolescents under the age of 18 years have not been determined. No data available.

How to use the drug Tagrisso

  • The drug is intended for oral use. Tablets should be swallowed whole with water and it should not be crushed, separated or chewed.
  • If the patient is unable to swallow the tablet, it can be mixed with 50 ml of filtered water.
  • If an infusion of a nasogastric tube is necessary, the same procedure as above should be followed but using a volume of 15 ml for initial dispersion and 15 ml for a sediment rinse.
  • The drug is usually applied once per day.

What to do in case of overdose?

  • Call 911 or go to the nearest local Medical Station right away.
  • Of course, you need to keep a record and bring a list of all medications you have taken, including prescription and over-the-counter.

What to do if I miss a dose?

  • If you forget to take a dose, take it as soon as possible.
  • But if it is almost like the next dose, skip the missed dose and take your next dose at the time as planned.
  • Do not take double the prescribed dose.

Note when taking drug Tagrisso

You should not use Tagrisso if you are allergic to osimertinib.

To make sure the medicine is safe for you, tell your doctor if you have:

  • Respiratory disorders;
  • Electrolyte imbalance (such as high or low levels of calcium, potassium, or magnesium in your blood);
  • Heart rhythm disturbances;
  • Personal or family history of long QT syndrome.

Tagrisso can harm an unborn baby or cause birth defects. Do not use if you are pregnant, and tell your doctor right away if you become pregnant. Use effective birth control while you are using this medicine and for at least 6 weeks after your last dose.

For men the father should use the course use contraceptive pills and for at least 4 months after you finish treatment., The baby may have birth defects.

This medicine may affect fertility (your ability to have children), whether you are a man or a woman. However, you should use birth control as recommended during and after treatment with medications.

Medication Side Effects

Get emergency medical help if you have  signs of an allergic reaction to Tagrisso:  hives; shortness of breath; swelling of your face, lips, tongue, or throat.

Call your doctor at once if you have:

  • Shortness of breath, even with mild exertion;
  • Feeling dizzy, like you might faint;
  • Swelling, rapid weight gain;
  • New or worsening lung symptoms - sudden chest pain or discomfort, wheezing, dry cough, feeling short of breath; or
  • Eye problems - changes in vision, watery eyes, increased sensitivity to light, eye pain or redness.

Your cancer treatments may be delayed or permanently stopped if you have certain side effects.

Common Tagrisso side effects may include:

  • Diarrhea;
  • Feeling tired;
  • Dry skin, rash;
  • Infection, or other problems with your nails or toenails.

What other drugs will affect Tagrisso?

Many drugs can interact with osimertinib . Not all possible interactions are listed here. Tell your doctor about all your current medicines and any you start or stop using, especially:

  • Antidepressants;
  • Cancer medicine;
  • An antibiotic or antifungal drug;
  • Antiviral drugs to treat hepatitis or HIV / AIDS;
  • Heart or blood pressure medicine;
  • Medication to treat a mental disorder;
  • Anticonvulsants;
  • Tuberculosis medicine.

This list is not complete and many other drugs can interact with Tagrisso. This includes prescription and over-the-counter medications, vitamins, and herbal products. Give a list of all your medicines to any healthcare provider who treats you.
