What is Keppra?

The antiepileptic Keppra is used as an adjunct therapy in adult patients with epilepsy for which current treatments are not effective in controlling partial epilepsy.

Medicines do not cure epilepsy, but only work to control seizures as long as the medication is used.

Indications for use of the drug

Keppra is indicated as monotherapy for the treatment of secondary generalized or partially onset seizures in newly diagnosed adults and adolescents 16 years of age with newly diagnosed epilepsy.

The drug is indicated as adjuvant therapy:

  • In the treatment of partial onset seizures with or without secondary generalization in adults, adolescents, children and infants from 1 month of age with epilepsy.
  • In the treatment of cardiomyopathy in adults and adolescents from 12 years of age with adolescent myocardial seizures.
  • In the treatment of primary general tonic-clonic convulsions in adults and adolescents aged 12 years with idiopathic generalized epilepsy.

Levetiracetam mechanism of action

Active ingredient Levetiracetam is an antiepileptic drug caused by excessive electrical activity in the brain.

How levetiracetam works is still unclear, but it appears to interfere with a protein called synaptic sac protein, which is found in the spaces between nerves and is involved in the release of chemical messengers from Nerve cells stabilize electrical activity in the brain and prevent seizures.

Dosage of medicine Keppra 500mg

  • Monotherapy: Adults & Adolescents Ages 16t: initiate 250 mg x 2 times / day & increase to 500 mg x 2 times / day after 2 weeks, can continue to increase by 250 mg twice daily each 2 weeks depending on response, maximum 1500 mg twice daily.
  • Combination treatment: Adults (≥ 18t) & adolescents (12-17t) weighing ≥ 50kg: initially 500 mg twice daily, response & tolerability may increase to 1500 mg x 2 times / day. Adjust the dose up or down to 500 mg twice daily every 2 to 4 weeks.
  • Elderly, ≥ 65t: dose adjustment according to renal function. Children 4-11 years old. & adolescent (12-17t) weight <50kg: initially 10 mg / kg x 2 times / day, response & tolerability may be increased to 30 mg x 2 times / day. Increase or decrease dose adjustment does not exceed 10 mg twice daily every 2 weeks.

Dosage adjustment

  • Children ≥ 50kg: same as adult dose.
  • Infants & children <4 years old: not recommended.
  • Renal impairment: dose adjustment according to ClCr Mild to moderate hepatic impairment: no dose adjustment required.
  • Severe hepatic impairment: reduction of half the daily maintenance dose when ClCr <70mL / min.

How to take the medicine Keppra

  • Take liquid pills and tablets by mouth as directed by your doctor.
  • Take twice daily with or without food. Crushing or chewing pills can cause a bitter taste.
  • Use this medication regularly to get the most benefit from it. To help you remember, take it at the same time every day.
  • Do not stop taking this medication without consulting your doctor. Your seizures may get worse when the medicine suddenly stops. Your dose should be gradually reduced.

Exercise caution before taking Keppra

You should not use the medicine if you are allergic to levetiracetam.

Tell your doctor if you have ever had:

  • Kidney disease (or if you are on dialysis);
  • Depression or other mood problems;
  • Mental illness or mental disorder;
  • Suicidal thoughts or actions.

Follow your doctor's instructions about taking anti-seizure medication if you are pregnant. Seizure control is very important during pregnancy, and seizures can harm both mother and baby.

You should not breast-feed while you are using this medicine.

Side effects of the drug Keppra

Report any new or worsening symptoms to your doctor, such as: mood or behavior changes, depression, anxiety, panic attacks, trouble sleeping or if you feel agitated, hostile, irritable, hyperactive or having thoughts about suicide or hurting yourself.

Call your doctor at once if you have:

  • Unusual changes in mood or behavior (unusual risk-taking, irritable, or talkative behavior);
  • Confusion, hallucinations, loss of balance or coordination;
  • Extreme sleepiness, feeling very weak or tired;
  • Problems with walking or moving;
  • Easy bruising, unusual bleeding;
  • Fever, chills, weakness, or other signs of infection.

Common medication side effects may include:

  • Dizziness, drowsiness, fatigue;
  • Feeling aggressive or irritable;
  • Loss of appetite;
  • Stuffy nose;
  • Infection.

Drug interactions 

Some medicines may make them less effective or cause side effects. Tell your doctor about all the medicinal products you are using such as prescription drugs, vitamins, herbs ...

Let your doctor know if you are taking anything that may make you sleepy, including:

  • Antihistamines such as cetirizine (Zyrtec or Alleroff);
  • Diphenhydramine (Benadryl, Sominex, Diphenhist, Wal-Dryl, Hydramine, Banophen, Dicopanol or Silphen);
  • Sleeping pills or anxiety medications such as alprazolam (Niravam or Xanax);
  • Diazepam (Valium);
  • Zolpidem (Edluar, Zolpimist, Ambien or Intermezzo);
  • Muscle relaxants;
  • Addictive pain relievers such as codeine.

Be sure to check the labels on all your medicines, especially those that cause allergies and colds as they may have ingredients that make you sleepy.
