What is Tracleer (bosentan)?

  • racleer lowers the blood pressure in your lungs, helping the blood pump your heart more efficiently.
  • The drug is used to treat pulmonary arterial hypertension (PAH). It improves your ability to exercise and prevents your condition from getting worse.
  • Tracleer pills are available only under a special program from a certified pharmacy. You must register in the plan and understand the risks and benefits of taking this medicine.
  • Bosentan may also be used for purposes not listed in the tracleer medication guide.

Medication indications

  • The drug Tracleer is used to treat pulmonary arterial hypertension (PAH, high blood pressure in the vessels that carry blood to the lungs). Bosentan is an endothelin receptor antagonist. It works by blocking the action of endothelin, a natural substance that causes blood vessels to narrow and blocks blood flow in people with PAH.
  • Pulmonary hypertension (group I-WHO) in patients with symptoms III-IV WHO class, to improve mobility and reduce the incidence of clinical worsening.

Contraindicated

Do not use Tracleer 125mg if:

  • Hypersensitivity to bosentan or any ingredient of the drug
  • you are pregnant or may be pregnant
  • you have liver problems
  • you are taking dasatinib, cabazitaxel, cyclosporine, or glyburide

Let your doctor know if you experience any of the conditions listed above.

Instructions for using the drug Tracleer 125mg 

Bosentan tracleer 125mg is a pill that is taken by mouth. It is usually taken with or without food, twice a day in the morning & at night.

Follow the directions on your prescription label carefully, and ask your doctor or pharmacist any part you do not understand. Take Tracleer exactly as directed.

Do not take more or less of it or take it more often than prescribed by your doctor. Your doctor will likely help you start with a low dose and increase your Tracleer dose after 4 weeks. Bosentan controls PAH symptoms but does not cure it. It may take 1-2 months or longer to feel the effects of the medicine.

Do not stop taking bosentan without first talking to your doctor. If you suddenly stop taking bosentan, your symptoms may worsen. Your doctor may reduce your dose slowly.

Tracleer dose : Start 62.5 mg twice daily x 4 weeks, then increase to maintenance dose 125 mg twice daily.

Usage : Can be used when hungry or full: Drink in the morning.

What should I do with Tracleer Overdose?

In case of an overdose, call your doctor immediately or if the patient shows signs of respiratory distress, call 115 medical emergency right away. Signs of an overdose may include: headache, stomach pain, vomiting, fast heart rate, fainting, dizziness, blurred vision.

What if I forget to take my medicine?

  • Take the missed dose as soon as you remember. However, if it is almost time for your next dose, skip the dose and continue your dosing schedule as directed.
  • Do not take a double dose to make up for a missed dose.

Note when taking Tracleer 125mg 

Some medical conditions can interact with bosentan. Tell your doctor or pharmacist if you have any of these medical conditions, especially the following:

  • you are pregnant, plan to become pregnant, or are breast-feeding
  • you can get pregnant
  • you are taking any prescription or nonprescription medicine, herbal medicine, or a special diet
  • you are allergic to drugs, foods, or other substances
  • you have a history of liver disease, abnormal liver function tests, heart disease (congestive heart failure), or blood problems (anemia)
  • fluid retention (abnormal swelling of the arms, hands, legs, or feet)
  • you drink alcohol or take medicines that can harm the liver (acetaminophen, methotrexate, isoniazid, some medicines for HIV infection).
  • Caution: Patients with moderate-severe hepatic impairment

Other drug interactions

Some drugs may interact with Tracleer. Tell your health care provider if you know about all of the medicines you are taking, especially the following:

  • Amiodarone, azole antifungal (fluconazole, itraconazole, ketoconazole, voriconazole), cobicistat, cyclosporine, diltiazem, HIV protease inhibitor (ritonavir), macrolide antibiotic (erythromycin), or tacrolimus as they can increase the risk. appendages of bosentan
  • Glyburide or rifampin for possible liver disease risks

What should I discuss with my doctor before taking medication ?

You should not use bosentan if you are allergic to it, or if you are pregnant or may become pregnant during treatment.

Some drugs can interact with Tracleer and should not be used at the same time. Your doctor may need to change your treatment plan if you use any of the following medicines:

  • cyclosporine;
  • glyburide.

To make sure bosentan is safe for you, tell your doctor if you have:

  • liver failure;
  • water retention;
  • Diabetes;
  • an autoimmune disorder such as rheumatoid arthritis or psoriasis; or
  • if you have had or will have a heart or kidney transplant.

Do not use Tracleer if you are pregnant or think you might be pregnant. This medicine can cause serious birth defects, especially if taken during early pregnancy. Tell your doctor right away if you miss a menstrual cycle or think you may have become pregnant during treatment.

You will need to have a negative pregnancy test before starting this treatment. You will be re-checked every month during your treatment, and 1 month after you stop taking this medication.

Even if you don't plan to get pregnant,  you are considered to be pregnant if:

  • you entered puberty (even if you haven't started your period);
  • have you ever had a hysterectomy or had your ovaries removed; or
  • you have not gone through menopause (you have never gone 12 consecutive months without a menstrual cycle).

While taking Tracleer pills and for at least 30 days after your last dose, you must use a highly effective form of birth control or two methods together.

  • A tubal ligation alone is an effective method of birth control.
  • An intrauterine device (IUD) alone is also an effective birth control method.
  • If you use birth control pills, implants, injections, dermal patches, or vaginal rings, you must use a form of contraceptive back-up barrier, such as a condom or diaphragm or cap. uterus. Always use a spermicide gel or insert together with a barrier form of birth control.
  • If you are only using one barrier method, you must use a second barrier method as a back-up. For example, using a diaphragm or cervical cap in addition to a condom, plus a spermicide gel or insert.
  • If your partner already has a vasectomy, you must still use a second method of contraception - either a barrier method or a form of hormones (birth control pills, injections, skin patches. , or vaginal ring).

Tracleer pills come with patient instructions on acceptable forms of birth control for use while taking this medication. Follow the instructions carefully. Ask your doctor or pharmacist if you have questions.

If you are a parent or caregiver of a female child taking this medication, talk to your child's doctor when you notice any signs of puberty (developing breast or pubic hair), even if The menstrual cycle has not yet begun.

Tracleer pills can reduce sperm count and may affect fertility in men (ability to have children).

It is not known whether bosentan passes into breast milk or if it could harm a nursing baby. You should not breastfeed while using this medicine.

Tracleer drug side effects

Get emergency medical help if you have  signs of an allergic reaction  : hives; shortness of breath; swelling of your face, lips, tongue, or throat.

In rare cases, Tracleer can cause a severe drug reaction that can affect many parts of the body. This type of reaction may start a few weeks after you start using this medicine. Seek medical treatment if you have new or worsening symptoms of:

  • Fever, flu symptoms, swollen glands, feeling weak or tired;
  • Swelling of the face;
  • A red or blistering skin rash, severe tingling or numbness;
  • Cough, chest pain, difficulty breathing;
  • Weight loss; or
  • Pain or burning when urinating, back pain, or swelling in your legs or feet.

Call your doctor at once if you have:

  • swelling in your legs or ankles, with or without weight gain;
  • pale skin, feeling short of breath, fast or irregular heartbeat, trouble concentrating;
  • a feeling of lightheadedness, like you might faint;
  • liver problems - nausea, vomiting, fever, upper stomach pain, fatigue, loss of appetite, dark urine, clay-colored stools, jaundice (yellowing of the skin or eyes); or
  • new lung problems - anxiety, sweating, pale skin, severe difficulty breathing, wheezing, panting, cough with sputum, chest pain, fast or irregular heartbeat.

Common side effects when you take Tracleer pills may include:

  • headache;
  • discharge (hot, red, or numb feeling);
  • arrhythmia;
  • low blood pressure, fainting;
  • athritis; or
  • cold symptoms such as stuffy nose, sinus pain, sneezing, sore throat.
