Indications for Treatment of Voriconazole

Vfend is a broad-spectrum antifungal drug of Azol group, indicated for the prevention and treatment of fungus in adults and children over 2 years old in the following cases:

  • Invasive Aspergillus infections, including pulmonary Aspergillus infection, complete, bronchial branches, bone marrow and organs.
  • Candidiasis in a patient with no leukopenia.
  • Invasive Candida infection: C.tropicalis, C.glabrata, C.kruisei ... including cases resistant to fluconazole and intraconazole.
  • Severe fungal infections were caused by Scedosporium spp. and Fusarium spp.
  • Candidiasis of the esophagus
  • Acute, life-threatening fungal infections, including mycelium and yeast.
  • Preventive treatment of fungal infections in high-risk subjects: marrow transplant patients, patients with recurrent leukemia.

Dosage and Administration Vfend 200mg Voriconazole

  • Recommended starting dose is: 200mg or 400mg, taken 12 hours apart during the first 24 hours.
  • After the first 24 hours the recommended dose is: 100 mg or 200 mg orally twice daily.

For both starting and maintenance dose, the dose used depends on your body weight and the type of infection.

  • Prophylactic dose in subjects at high risk of fungal infection: use Vfend 200 at a dose of 9mg / kg twice daily, but not to exceed 350mg x 2 times / day.

Preventive treatment should be initiated on sabbatical and can be maintained for up to 100 days. Prophylactic treatment should be as short as possible depending on the risk of developing an invasive fungal infection (IFI) according to neutrophils or impaired immunity. The prophylactic dose lasts only up to 180 days from implantation in case of continued immunodeficiency or GVHD.

Contraindicated for use

  • Patients with severe hepatic impairment.
  • Concomitant administration with substrates of CYP3A4, terfenadin, astemizol, cisapride, pimozide or quinidine because increased plasma concentrations of medicinal products can lead to prolonged QT and the rare occurrence of torsion.
  • Concomitant administration with rifampicin, carbamazepin and Phenobarbital because medicinal products are capable of significantly reducing voriconazole plasma concentrations.
  • Concomitant administration of the recommended dose of voriconazole with efa-virenz 400mg once daily or higher, or ritonavir (400mg and more twice daily). Because these drugs significantly reduce the plasma voricoazole concentration.

Vfend Precautions and Interactions

  • Use with caution when using Vfend-200 in patients with a history of hypersensitivity to azole drugs, patients with cardiovascular diseases: arrhythmia, cardiomyopathy.
  • Patients with moderate and mild liver dysfunction: it is necessary to closely monitor the patient's liver function during the entire period of use, and to shorten the duration of treatment as soon as possible in these patients.
  • Patients with electrolyte disorders: need evaluation and adjustment before taking the drug.

Interactive

Drugs that may interact with Vfend include: Ergot alkaloids (such as ergotamine, dihydroergotamine), sirolimus, a combination HIV drug (such as efavirenz / emtricitabine / tenofovir ).

  • Combination drugs that can affect heart rate (QT prolongation) include amiodarone, dofetilide, pimozide, procainamide, quinidine, sotalol, and macrolide antibiotics (such as erythromycin).
  • Voriconazole may slow the elimination of other drugs from the body such as alpha blockers (alfuzosin, silodosin, tamsulosin), oral contraceptives, cisapride, dronedarone, eletriptan, eplerenone, lurasidone, ranolazine, statins (such as lovastatin, simvastatin). , ticagrelor, tolvaptan. Substances that can affect the elimination of voriconazole from the body include sedatives (mephobarbital, phenobarbital), carbamazepine, enzalutamide, primidone, rifamycins (such as rifabutin, rifampin), ritonavir, alcohol.

Caution for pregnant and lactating women

  • Pregnancy: The drug should not be used during pregnancy, except in the case of a life-threatening emergency and the benefit to the mother outweighs the potential risk to the fetus.
  • Breast-feeding women: no data available should not be used while breastfeeding. Breastfeeding should be stopped when the mother must use the drug.

Undesirable Effects when using Vfend
