What is Erlonat medicine?
The drug Erlonat contains an active ingredient erlotinib 150mg; The drug is mainly used to treat non-small lung cancer in patients when chemotherapy is ineffective.
The drug works by blocking the actions of certain proteins in the body to hinder the growth of cancer cells and slow their spread in the body.

Indications for drug treatment
Treatment of patients with locally advanced or metastatic non-small cell lung cancer (NSCLC).
Local advanced treatment of pancreatic cancer patients, unable to remove or metastasize.
The drug works by slowing the growth of cancer cells. It binds to a certain protein (EGFR) in some tumors.
Contraindications to the use of drugs
Do not use the drug for pregnant and lactating women.
Do not use for children under 18 years old.
Do not use the drug in patients allergic to erlotinib ingredients or to any of its ingredients.

The standard dosage of the Drug Erlonat 150mg erlotinib
Non-small cell lung cancer: The daily dose of the drug Erlonat is 150 mg orally at least one hour before or two hours after eating.
Pancreatic cancer: The recommended daily dose of the drug is 100 mg orally at least one hour before or two hours after meals, in combination with gemcitabine.
Special Dosage Instructions: Concomitant use of CYP 3A4 preparations and modulators may require dose adjustment.
How to use erlotinib 150mg?

Avoid exposure to direct sunlight for long periods of time; wear a hat, protective clothing, sunglasses and sunscreen. Exposure to sunlight increases the risk of rashes during Erlonat treatment.

Stop using Erlonat and call your doctor at once if you have:
Sudden chest pain or discomfort, wheezing, a dry cough, feeling short of breath;
Stomach pain, fever, chills, coughing up blood;
Severe continuous nausea, vomiting, or diarrhea;
Eye pain or irritation, vision problems;
Heart attack symptoms: chest pain or pressure, pain spreading to your jaw or shoulder, nausea, sweating;
Signs of stroke: sudden numbness or weakness, severe sudden headache, slow speech, problems with vision or balance;
Kidney or liver problems: little or no urinating; swelling, rapid weight gain; confusion, nausea, vomiting, dark urine, or yellowing of the skin or eyes;
Serious skin reactions: fever, sore throat, swelling in your face or tongue, burning in your eyes, skin pain, followed by a red or purple skin rash that spreads and causes blistering and peeling.
Patients using the drug Erlonat may experience undesirable side effects such as:
Nausea;
Vomiting;
Stomach disorder;
Diarrhea;
Dry skin;
Loss of appetite;
Mouth sores.
More serious side effects such as:
Easy bleeding or bruising;
Jaundice;
Unusual fatigue;
Signs of infection;
Black, bloody or tarry stools;
Eye irritation or pain;
Vision or speaking problems;
Shortness of breath.
Warnings when taking the drug Erlonat
Immediately go to your nearest emergency department if you have an allergy. Symptoms commonly associated with such a reaction include difficulty breathing or swallowing, chest tightness, swelling, skin rash, and hives.
The drug Erlonat is not always suitable for all patients. Always consult your doctor before using this medicine if you are pregnant, nursing, attempting to conceive.

To make sure Erlonat is safe for you, tell your doctor if you have:
Lung or breathing problems (except lung cancer);
Kidney disease;
Liver failure ;
History of stomach bleeding;
Eye problems;
If you become dehydrated;
If you also take warfarin (Coumadin, Jantoven).
Do not take Erlonat 150 if you are pregnant. It can harm an unborn baby. Use effective birth control while you are taking this medicine and for at least 1 month after your last dose.
It is not known whether the medicine passes into breast milk or whether it could harm a nursing baby. You should not breast-feed while taking the medicine.
Drug interaction Erlonat
Erlotinib is metabolized in the liver by human hepatic cytochromes, mainly CYP3A4.
Interactions may occur when drugs inhibit or induce the CYP3A4 enzyme metabolism.
Strong inhibitors of CYP3A4 activity
The drug is less metabolized in the liver and increases the concentration of erlotinib in plasma.
Inhibition of CYP3A4 metabolism with ketoconazole (200 mg orally twice daily for 5 days). This leads to an increase in Erlonat drug concentration of 69% of the maximum plasma concentration (Cmax) when compared with the drug.
When the drug Erlonat 150mg concomitantly used ciprofloxacin, an inhibitor of both CYP3A4 and CYP1A2. Exposure to Erlonat (AUC) and Cmax increased by 39% and 17%, respectively.
Therefore, caution should be exercised when administering the drug with potent CYP3A4 inhibitors or a combination of CYP3A4 / CYP1A2. In these cases, the dose of Erlonat drug should be reduced if toxicity is found.
Strong inducers of CYP3A4 activity
These drugs increase drug metabolism Erlonat and significantly reduce the drug concentration in the blood plasma.
When rifampicin is used concomitantly (600 mg orally every day for 7 days). This resulted in a 69% reduction in mean Erlonat AUC, after erlotinib 150 mg, compared to Erlonat alone.
Pretreatment and combination of rifampicin with a single 450 mg Erlotinib dose. This resulted in an average drug concentration (AUC) of 57.5% following a single dose of erlotinib150 mg.
For patients requiring concomitant therapy with Erlotinib and CYP3A4 inducers. Dosage increases to 300 mg or 450 mg are indicated, however safety monitoring is required.
Drugs that affect the absorption of Erlonat from the intestine
The solubility of Erlotinib 150mg is dependent on pH. The solubility of the drug Erlonat decreases with increasing pH.
Medications that alter the pH of the upper digestive tract (GI) can alter the solubility of medications.
Concomitant use of the drug Erlonat 150mg with omeprazole, a proton pump inhibitor. This drug reduces erlotinib (AUC) and Cmax exposure by 46% and 61%, respectively.
Concomitant administration of Erlotinib with ranitidine 300 mg, an H2 receptor antagonist, reduces Erlonat (AUC) and Cmax exposure by 33% and 54%, respectively.
Therefore, concomitant use of drugs that reduce gastric acid production with Erlotinib 250mg should be avoided if possible.
Interaction with coumarin-derived anticoagulants
The drug warfarin, leading to increased bleeding rates, has in some cases been reported in patients taking Erlonat.
Patients on coumarin-derived anticoagulants should be monitored regularly.
The combination of erlotinib 150mg and a statin may increase statin-induced myopathy potential. Side effects include rhabdomyolysis.
